Introducing SpaSphere Forms: Free, Unlimited Online Forms for Estheticians
SpaSphere Forms

Your client already told you about her medications, her allergies, the retinol she stopped last week and the peel that went badly two years ago. That shouldn't live in a form tool in one tab while her client profile lives somewhere else.

Today we're launching SpaSphere Forms: online forms built for estheticians, and free. Unlimited forms, unlimited submissions, no card. Every answer lands on the client's record, where you will actually look for it before her next treatment.

SpaSphere Forms is free, with no limit on forms or submissions. No card needed.

What SpaSphere Forms is

SpaSphere Forms is a free online form builder for estheticians. You build intake, consultation and agreement forms, send them to clients, and every submission is saved on that client's profile. There is no limit on how many forms you create or how many submissions you receive, and you don't need a card to start.

It is not a trial and not a stripped-down preview. It is the forms part of SpaSphere, on its own, for as long as you want to use it.

Built for the treatments you actually do

You don't start from a blank page. The template library has 15 ready-made forms and 16 sections, all editable:

  • Intakes by treatment: Basic Facial, Acne Consultation, Chemical Peel, Microneedling / Microchanneling, Dermaplaning, Facial Wax, Brazilian Wax, Lash Extension, Brow Lamination / Tint, and Body Contouring.
  • New and returning clients: a New Client Intake and a Returning Client Update, so a regular only tells you what changed.
  • Agreements: Membership Agreement, Package Consent and Treatment Plan Agreement.
  • Sections you can drop into any form, including Medical History, Allergies & Sensitivities, Medications & Supplements, Skin History, Pregnancy & Breastfeeding, Photo Consent, Consent & Acknowledgement, and Client Policies.

Build your own from nine question kinds: heading, short answer, long answer, dropdown, single choice, multiple choice, paragraph, photo or file, and signature. Save a section once and reuse it across forms. A choice question can open a follow-up question, so "Yes" to "Are you taking any medications?" asks her which ones.

Already have forms? Bring them

Most estheticians already have an intake form, often a PDF that has been edited for years. Upload that PDF and SpaSphere turns it into an editable SpaSphere form. PDF import is included on the free plan. Check it over, adjust what you want, and send it.

Signatures and photos, on her phone

Clients fill out your forms on any phone, tablet or computer. No app to download and no account to create.

  • E-signatures. She draws or types her signature. It is saved with the submission, along with the time, her IP address and her browser, and you see it when you open the submission.
  • Photos and files. Forms can ask for intake documents, a consent or waiver file, a client picture, or before and after photos. Files are virus-scanned and land on her Files tab.

Every submission lands on the client's record

This is the part that changes your day. Every form on SpaSphere Forms starts with a short Your Info section: first name, last name, email and phone. When a form comes in, SpaSphere matches it to an existing client by email. If she's new, and the form is open to new clients or you emailed her the link, a new client is created for you.

So her answers are not a row in a spreadsheet. They sit on her profile, under Intake Forms, next to her photos and files. When she comes back in six weeks, you open her profile and her medications, sensitivities and skin history are already there, ready for your assessment.

You also see the latest submissions and counts on your Forms dashboard. When a form comes in, you get an email and an app notification. Your client gets an email confirming you received it.

Getting forms to clients

  • Copy a link and send it however you already talk to clients.
  • Email it to a client straight from SpaSphere.
  • Print a waiting-room sign with a QR code that opens your forms page, so a walk-in can fill hers out from the chair.

The SpaSphere staff app for iPhone and Android does the same work: build forms, start from templates, send by email or link, and read submissions, signatures included.

How it compares to the tools you may be using

If you're on a general form tool today, two numbers worth checking, as of October 2026:

  • Jotform's free Starter plan allows 5 active forms, 100 submissions a month and 10 signed documents a month, with Jotform branding included (Jotform pricing). Its lowest paid plan, Bronze, is $39 a month.
  • Google Forms is free. Its question types don't include a signature, and for a file upload question, "responders need to sign in to a Google Account" (Google Docs Editors Help). That's worth knowing before you ask a client to upload a photo.

Both are good general tools. SpaSphere Forms is narrower on purpose: it is built for an esthetics practice, with no cap on forms or submissions, and it keeps every answer with the client it belongs to.

When you're ready for more: SpaSphere Pro

SpaSphere Forms is complete on its own. When you want the rest of your practice in the same place, SpaSphere Pro adds online booking and your calendar, checkout and payments, invoices, reminders and automations, a full website, memberships, marketing, SOAP notes and AI features.

It also connects your forms to your bookings. Link a form to a service, and it goes out automatically with the booking confirmation and reminders. Your Chemical Peel Intake reaches every peel client before she arrives, and her answers are on her profile when you open her SOAP note.

SpaSphere Pro is one complete plan: 30 days for $1, then $139/month, no feature tiers. Cancel anytime. Upgrade and everything you've already set up is ready to go.
